We are looking foran enthusiastic, self-motivated and experienced Clinical Pharmacist to join our friendly team. We are a forwardthinking and dynamic practice that embraces the value of a multi-disciplinaryteam with an establishedpractice based pharmacist role embedded in the team. Due to retirement, we are now looking for a pharmacist to continue to develop and manage the role of the pharmacist within the practice. An Independent Prescriber is preferred.
Main duties of the job
This is very much a patient-facing role as well as providing primary support to general practice staff with regards to the safe and effective use of will be plenty of opportunity to utilise your clinical skills to improve the health of our practice population.
The role will be based in our practice alongside our clinicians including pharmacists deployed from our PCN.
The ideal candidate will be enthusiastic and adaptable to change, with excellent clinical skills and a good level of competence in the use of IT. Candidates should exhibit good organisational skills and the ability to communicate effectively.

Archwood is well established, family friendly practice with a CQC rating of Good. We care for circa 7900 patients in the Woodley, Romiley and Bredbury areas of Stockport.
The team includes 4 GP Partners, 2 salaried GPs, an ANP, Practice Pharmacist, Nursing team, Practice Manager and a full complement of reception and admin staff. The practice works closely with our Primary Care Network (PCN) and local community services colleagues to provide excellent healthcare to our patients.
The health and wellbeing ofstaff and patients is always a main priority and the Partners work hard toprovide a supportive and rewarding workplace. We are a Training practice for GPs and are committed to training anddevelopment for all staff.
Woodley Health Centre is a modern purpose-build building with excellent facilities for staff and patients, including free on-site parking. Close to junction 25 on the M60, Woodley has excellent road andpublic transport links into Manchester City Centre and a short drive from the beautifulPeak District.

Job responsibilities
You will be working as part of the clinical team in a patient facing role, committed to optimising prescribing, improving patient care, and managing long term conditions.
Responsibilities will include:
1. Undertake medication reviews with patients face-to-face and over the phone. This will include patient education, prescribing and monitoring as per guidelines and within professional competencies.
2. Proactively manage patients with complex polypharmacy. Implementown prescribing changes (as an independent prescriber) and order relevantmonitoring tests.
3. Lead on the implementation and management of a robust medication review process.
4. Work closely with the GPs, other practice staff and healthcare teams ( community pharmacy) to answer all medication related enquiries. Suggesting and recommending solutions.
5. Update patient medication records post hospital discharge and outpatient appointments and advise GP of all issues that may arise.
6.Provideleadership to the general practice team to ensure the practice is compliantwith CQC standards where medicines are involved. Undertake risk assessment andmanagement and ensure compliance with medicines legislation.
7. Influence and implement changes promoting safe and cost effective prescribing.
8. Ensure all MHRA drug alerts are actioned and recorded appropriately.
9. Work alongside PCN Clinical Pharmacist team and implement work initiated by them into practice.
10. Contribute to attainment of prescribing targets including Quality Outcome Framework (QOF) and Investment and Impact Fund (IIF)
11. Manage time effectively to ensure all targets are met.
12. Complete or implement clinical audit work to improve prescribing safety and improve patient care.
13. Take an active role to support practice initiatives.

Qualifications
Essential
1. Registered with relevant pharmacy regulator (GPhC)
2. Degree in pharmacy, MPharm/BPharm
3. Independent prescribing qualification

4. Post Graduate diploma in Clinical Pharmacy
5. Experience working in General Practice
6. Clinical Systems trained (EMIS, Docman)
