Job overview
The Living Well Practice Development and Practice Standards Lead role supports both effective patient safety and quality through embedding clinical governance processes in a programme that requires a continual improvement approach towards patient pathways and clinical systems/processes. This role ensures a consistent approach in the delivery of the Community Mental Health Framework ensuring transformation of services is completed in line with legislation and evidence-based practice.
Main duties of the job
* To work closely with the Professional Leads and Heads of Nursing as the senior clinical role models and leaders for Derbyshire Living Well
* To lead in the continued design, development, and advancement of the Derbyshire Living Well practice model
* To embed a culture of co-production and ensure the voice of lived experience is integral to the Derbyshire Living Well practice model
* To lead on developing practice standards to ensure evidenced-based best practice is driving effective clinical productivity and quality outcomes for patient care within Derbyshire Living Well services
* To act as a link between internal services within DHCFT and external partners in developing practice, pathways, and services
* The post holder will demonstrate expertise in project management and leadership, to continually develop, deliver and monitor the Derbyshire Living Well practice model, designed around what matters to individuals and that people are at the centre of the support they receive. They will work innovatively and embrace new ways of working to improve mental health outcomes for people and their communities, and ensure the service is always at the forefront of best practice
* The post holder will have a deep understanding of the importance of working collaboratively in effective partnerships with health, community and third sector organisations, adult social care and commissioners

Person specification
Attainment/Qualifications
* Registered Nurse (RNMH, RNLD), Occupational Therapist or Social Worker and current registration with relevant professional body
* Evidence of continuous professional development
* Practice supervisor/assessor completion or equivalent; Completion of preparation for mentorship/fieldwork educator qualification or equivalent
Skills and Knowledge
* Able to demonstrate commitment to high quality care and service provision; Proven track record of achieving objectives
* Excellent leadership skills; Flexibility - adapts to ensure achievement of objectives within changing environment; Able to work collaboratively with teams and leaders; Ability to manage conflict; Innovative thinker with the ability to cut through barriers to change; Ability to motivate and influence others
* Knowledge of current national policies in relevant area of health & social care; Ability to interpret national guidance and research findings to develop best practice standards
* Knowledge of IT, including Microsoft Office and Power Point; Ability to gather and analyse data and produce written reports for a range of audiences and purposes
* Ability to use a range of problem-solving methods to identify solutions to complex situations; Ability to work collaboratively and effectively across agencies and secure successful interagency and partnership working; Excellent communication skills
Experience
* Broad range of experience in mental health, including community; Significant experience in a management/leadership role; Experience of managing change and associated behaviours; Experience of successful change management and service development; Experience of working in health or social care services at a senior level
* Experience of teaching and assessing; Experience in implementing quality improvement and influencing changes in clinical practice; Experience of developing policies and procedures; Experience of service delivery that comply with quality and performance schedule
* Experience of working with external stakeholders, such as commissioners, Local Authority, VCSE and managing delivery of safe services
* Experience of co-production
