Social Workers required - working across the Community Learning Disability Team and Intensive Support Team (co-located within one team).

Bolton Council is currently seeking dedicated Adults Social Workers to join a multi-agency Community Learning Disability Team and Intensive Support Team.
We are offering the opportunity to join a focused and dynamic team who are involved in the assessment and development of a wider range of services in Bolton to meet the current and future needs of local people who have a learning disability.
We are a large multidisciplinary team involved in complex areas of work and so there are many opportunities to be involved in formal decision making forums and court processes.
We are looking to recruit 2 enthusiastic and motivated individuals who have a passion for working with adults with Learning Disabilities, understanding their needs, aspirations and supporting them to enhance their quality of life.
The work will involve working with adults with a learning disability within the community to assess and support plan, as well as working with individuals with a learning disability whose needs present as a risk of significant harm to self, or others. This may include individuals who have offending histories, who have, or are at risk of having contact with the Criminal Justice System, or at risk of admission to hospital or in out of borough placements due to the complexities of their needs.
You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and complex decision making.
Key Duties:
1. Care Act assessments and strengths based support planning, taking into account available resources, universal and voluntary services.
2. Carers assessments.
3. Capacity assessments and Best Interest decision making.
4. Continuing Health Care assessments.
5. Inter disciplinary and multi-agency working.
6. Safeguarding investigations.
7. Supporting individuals to move into alternative accommodation.
8. Preferred experience of resettlement from out of area placements back into the local area.
9. Experience of report writing such as Ministry of Justice Tribunal reports.
10. Social circumstances reports for Mental Health Tribunals.
11. Experience of Hospital Discharges.
12. Reports for Community Treatment Reviews.
13. Court of Protection applications.
Qualifications and Experience:
1. Qualified Social Worker.
2. Experience of complex work with individuals with a learning disability in a statutory setting.
3. Experience of using liquid logic systems would be preferable.
