Job summary We are transforming our Community Stroke Service to provide individualised stroke rehabilitation to all stroke survivors within Worcestershire. Our vision is to roll out the national Integrated Community Stroke Service model. If you are passionate about stroke and improving quality of lives, then this job is for you. Our integrated and needs-based approach means that you will work with both inpatients and stroke survivors in their own homes. Main duties of the job Day to day, with your SLT colleagues, you will be undertaking comprehensive assessments and treatment of stroke patients with speech, language, voice and fluency difficulties. This will be for inpatients and the community setting. You will be supported to achieve your dysphagia competencies following one year of embedding into the team. You will formulate individual programmes of rehabilitation using clinical reasoning and utilise a range of treatment skills, targeting realistic and measurable, patient-led goals. It is also expected that you will offer support and education to carers/families and the wider team. You will input into the weekly MDTs in which the team will discuss the progress of patients and identify goals that are achieved and ongoing. You will also need skills in advising, educating, skills in motivating and counselling and providing patient-centred care.
