The service is looking for a highly motivated individual to work alongside the CNWL Brent Learning Disability Community Health Team and assume the role of the Transforming Care programme lead and case manager locally within the Team.

The purpose of this role is to support the commissioning of safe and effective services for people with learning disabilities. This involves supporting discharge from the hospital and the oversight of commissioned community packages of care, thereby preventing hospital admission.
It will involve participating in assessing, planning, and implementing proactive care and case management for Brent's population with a Learning Disability or autism. The post holder will be the main point of contact for health and social care colleagues, providers, service users, family, and carers with the appropriate placements and facilitate the care system to move people through the system regarding the Transforming Care Concordat.

* Experience in assessment, planning, implementation and evaluation of care and treatment plans.

Education and Qualifications
Essential
* Professional qualification relevant to LD e.g. Registered Nurse Learning Disabilities, social worker, Therapist, Clinical Psychology, AHP's.
