Main duties of the job As a Social Prescribing Lead your responsibilities will vary. You will Provide Leadership and management responsibilities for the social prescribing co-ordinators on site. Complete initial assessments with patients referred to and eligible for the social prescribing service. Assessing their current wellbeing and identification of what matters to them. Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing. Experience of using SystmOne Experience offering an information, advice and guidance service in a health, care or similar setting Experience of working within a prison environment or working closely with patients with challenging and complex needs.
