We have a fantastic secondment opportunity for a 2-year fixed term, 0.8fte, band 7 Learning Disability Nurse within the Intensive Support Function of our Community Learning Disability Team, based in East Cheshire. As this post is fixed term, secondment opportunities can be discussed with your current manager.
You will be joining a team where person-centred care is at the forefront of everything that we do.
You will be responsible for delivering high-quality clinical care and supporting the clinical leadership within the Intensive Support Function and the community nursing team, ensuring all work is needs-led, evidence-based and in line with current legislation.
You will also support the leadership and development of services in line with the workstreams that are the focus for the care group, which includes mental health, forensic, physical health, NDD, and cognitive impairment.
CWP hosts CANDDID, the Centre for Autism Neurodevelopmental Disorders and Intellectual Disabilities, which carries out research, education, and training, and all clinicians are encouraged and supported to develop research and QI skills.

The post holder will carry a caseload of clients within the Intensive Support Function who have learning disabilities and the most complex needs, where a very high level of clinical expertise is required to deliver care that will reduce the risk of placement breakdown and admission to inpatient services under the Mental Health Act.
The post holder will act as a transformational role model/guide to deliver specialist learning disability assessment, intervention, and person-centred outcomes.
They will also be responsible for providing clinical supervision and appraisals of nurses within the team and supervise students while on placement with the team.
CWP provides health and care services for local people, including mental health, learning disability, community physical health, and all-age disability care – including the provision of three GP surgeries.
