Position Name: NACDD Public Health AmeriCorps Service Member
Position Type/Hours: Three Quarter Time/1200 hours
Service Location: Valley of the Sun YMCA
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control.
To enhance public health resources across the country and support state and local public health settings respond to and recover from the COVID-19 pandemic, the Public Health AmeriCorps (PHA) program supports the recruitment, training, and development of the next generation of public health leaders.
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
* Provide direct support for outreach and promotion of health services.
* Conduct Health Related Social Needs or Social Determinants of Health assessments and coordinates referrals to social service agencies.
* Design whole-person health plans for individuals and families.
* Support client enrollment in medical and/or behavioral health services.
* Serve as an advocate, role model and guide in individual’s journey for stability and wellness.
* Implement and host social engagements which promote social connectivity.
* Provide culturally appropriate and accessible health education and information.
* Provide direct services for physical activities and administer health screening tests.
* Educate individuals and their families on the importance of lifestyle changes.
* Maintain accurate records, statistical data and program reports as required.
* Attend staff meetings and trainings as required.
* Follow all YMCA policies, rules, regulations, and procedures.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
* In-person service hours; Must have availability on evenings and weekends.
* Service hours are to be completed Monday-Sunday, typically between the hours of 8:00am-5:00pm.
* A maximum of 20% of service hours may be training, education, or other similar approved activities.
* A maximum of 10% of service hours may be fundraising activities.
DESIRED SKILLS & COMPETENCIES:
* Experience collaborating with a variety of community service providers.
* Knowledge of, and previous experience with, diverse populations preferred.

REQUIRED COMMITMENT:
* Service members are expected to complete 1200 service hours within a one (1) year timeframe.
* The service year begins 02/03/2025 and ends 02/02/2026.
ORIENTATION AND TRAINING:
* Members must participate in pre-service orientation, PHA-specific training, and program-specific training.
EVALUATION AND REPORTING:
* Complete quarterly assessments of the PHA Program.
* Complete monthly reflection forms.
* Participate in quarterly performance evaluation and reviews.
* Submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
* Be at least 18 years of age at the commencement of service.
* Have a high school diploma or its equivalent.
* Be a citizen, national, or lawful permanent resident alien of the United States.
* Satisfy the National Service Criminal History Check eligibility criteria.
* Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
* Must comply with all Host Site vaccination requirements.
* Complete all safety and compliance trainings within 90 days of hire date.
