Responsibilities
Conduct home visits and assessments to gather information about clients' needs and circumstances.
Develop and maintain strong relationships with clients, stakeholders, and community organizations.
Document findings from visits accurately and timely to ensure proper record-keeping.
Assist clients in accessing services and resources appropriate to their individual situations.
Collaborate with multidisciplinary teams to create and implement effective support plans.
Monitor progress and outcomes for clients to ensure their needs are being met effectively.
Provide guidance and support to clients, helping them navigate challenges they may face.
Requirements
Proven experience in social work or a related field, preferably in a visiting role.
Strong interpersonal and communication skills, both written and verbal.
Ability to work independently and manage a caseload effectively.
Compassionate and empathetic attitude towards clients and their challenges.
Proficiency in report writing and documentation practices.
Strong organizational skills and attention to detail.
Valid driver’s license and willingness to travel to various locations for home visits.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £20.00
Location: Enfield, London
