Anexciting opportunity has arisen for an appropriately skilled and experiencedregistered nurse to join Kirklees Health and Care Partership's ContinuingHealthcare (CHC Team) as a lead nurse for the physical disability CHC Team. Although direct experience of working in a CHC team is notessential, candidates must be able to demonstrate knowledge and experience ofthe CHC legislation and process and have experience of working within the physical disability field of practice.
Assessment,including risk assessment, and case management skills are essential.
Youshould have excellent communication and interpersonal skills with the abilityto produce clear and concise written reports.
Full-timeand part-time hours considered.

Main duties of the job
UndertakingContinuing Healthcare (CHC)/Funded Nursing Care (FNC) assessments and reviewsfor patients across all aspects of care. This necessitates a high level ofassessment skills requiring tact, diplomacy and the ability to liaiseeffectively with colleagues, health care professionals, service users' carersand family members.
Complete MCA assessments and Best Interest decisions where appropriate. Workwithin the DoLS framework, identifying and responding to the need for DoLSauthorisation this this may include working with legal professionals and TheCourt of Protection.
Offering continuing support and advice to family and carers in complex andsensitive situations, necessitating a high level of interpersonal, andreassurance skills; supporting the local resolution process in the case ofcomplaints/appeals/retrospective cases and providing information on a timelybasis.
Developing and maintaining effective and appropriate communication networkswith health and social care professionals and act as a resource to them.
Working with relevant health professionals, CHC colleagues, Business SupportStaff, Senior Leads and other Support Staff within the CHC team to ensure aseamless and efficient service is delivered.

Exercising professional judgement as necessary when analysing andcommunicating complex and sensitive information on behalf of the CHC serviceand application of the associated criteria. Interpreting national andregional guidance relating to the implementation of the CHC NationalFramework and subsequent developments within the CHC service.
Decision making with regards to multi-disciplinary team CHC assessments todetermine eligibility.
Supporting Community Matrons/District Nurses in their case management rolesfor complex cases that are eligible for continuing healthcare. Deputise forthe CHC Team Leader when required.
Deliver against organisational objectives, achieving quality outcomes,prioritising own workload and working to tight deadlines.
Plan, organise and implement reviews and assessments, to co-ordinate withLocal Authority, family and providers
Ensuring there is a holistic care plan in place, including the production andco production when required with Health and Care partners.
Ensure personal health budgets are actively promoted with service users andtheir representatives and work closely to encourage a seamless delivery.
Identify risk of abuse and need for protection from harm to self or othersand report in line with local safeguarding routes and governancearrangements.
Ensuring timely "End of life" provision and reviews, following thenational guidance for clients Fast Tracked through the CHC system.
Providing specialist nursing advice to secondary care, primary care, nursinghomes etc to facilitate the continuing healthcare process.
Ensure policies are implemented and adhered to whilst identifying andproposing changes for improvement to practices and procedures in own area.Required to implement changes to policies and procedures.
Approving a package of care in line with Place-based Partnership standingfinancial instructions and agreed thresholds.
Continually strive for delivering project/function outcomes, value for moneyand greater efficiency
Agreeing eligibility and will make financial decisions within the Place-basedPartnership standing financial instructions.
Delivering formal and non-formal education programmes to Community andhospital staff, including nursing/residential homes, GPs, community nursingand therapy staff relating to the assessment and management of CHC patientgroups in line with the latest research, evidence and Department ofHealth/NICE guidance.
Maintaining and updating clinical records in a timely manner and in line withlocal and national policies

Person Specification
Qualifications
* Registered Nurse (RGN/RMN/RNLD/RSCN)
* Educated to degree level or equivalent in experience
* Current and relevant NMC registration
* Post Graduate qualification or specialist training i.e. Community Nursing
Experience
* Minimum 2 years post qualification experience as a Registered Nurse
* Evidence of on-going professional development
* Significant experience of both community and hospital working and possess a sound understanding of the NHS CHC national guidance.
* Demonstrable evidence of the use of key performance indicators.
* Experience of managing change and supporting change management processes.
* Extensive knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards
* Up to date knowledge of the national nursing agenda
