Worcester City PCN is part of Vertis Health, a federation of GP surgeries and Primary Care Networks in Worcestershire, serving over 600,000 NHS patients. We focus on reducing administrative workload for GPs, allowing them to spend more time with patients. The Role:
We are seeking a committed Nurse Associate to join our Primary Care Network. In this role, you will support the delivery of high-quality care within our GP practices, working alongside the clinical team to provide comprehensive and patient-centred care. This is an excellent opportunity to contribute to the health and wellbeing of our community.
Key Responsibilities:
1. Work with the multidisciplinary team to deliver comprehensive care across all ages.
2. Assist with the assessment, planning, implementation, and evaluation of nursing care plans.
3. Conduct routine health checks, administer medications, and assist with minor procedures.
4. Perform diagnostic tests and assist in the collection and interpretation of data, including vital signs and patient histories.
5. Provide health education and advice to patients and their families, promoting healthy lifestyles.
6. Maintain accurate and up-to-date patient records, ensuring confidentiality.
7. Coordinate and facilitate referrals to specialist services, ensuring timely access to care.
8. Participate in quality improvement initiatives to enhance patient care.
9. Keep up-to-date with developments in healthcare and attend relevant training sessions.
What We’re Looking For:
1. Valid nursing qualification and registration as a Nurse Associate.
2. Experience in a primary care or community healthcare setting is desirable.
3. Strong understanding of clinical procedures, protocols, and evidence-based practice.
4. Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
5. Good organisational skills and the ability to manage a busy workload effectively.
6. Flexibility to work occasional Saturday shifts as required.
7. Proficiency in using electronic health record systems and other relevant software.
8. Commitment to professional development and continuous learning.
