Rate of Pay: £17.59 - £21.89 per Hour
Summary:
The Sustainable Housing Support Officer is responsible for delivering proactive housing support to vulnerable clients in the Private Rented Sector (PRS), with a focus on tenancy sustainment. This role involves supporting individuals with multiple and complex needs to maintain accommodation, coordinating services with internal and external partners, and ensuring compliance with legislation, safeguarding policies, and Council procedures. The officer will carry a caseload, provide advice and practical assistance, and contribute to delivering the Council's strategic aim of ending rough sleeping.
Responsibilities:
* Case Management & Client Support: Manage a caseload of clients, delivering tailored Sustainable Housing Support Plans to assist them in maintaining tenancies and developing independent living skills. Provide advice and practical support to meet individual client needs while ensuring all aspects of support work comply with relevant legislation and safeguarding policies.
* Collaboration & Stakeholder Engagement: Work closely with internal and external partners, including Housing Advisory Service, Adult Social Care, NHS, police, and community organizations, to improve outcomes for clients.
* Data Management & Reporting: Ensure accurate and timely entry of client information into case management systems, producing reports and analysis as required by the Team Manager.
* Service Delivery & Continuous Improvement: Support the delivery of an efficient and person-centered service through continuous review and adaptation of support plans. Participate in training and team workshops to promote good practice and improve service delivery.
Qualifications:
* High level of education or equivalent demonstrable experience in housing support or a related field.
* Evidence of Continuous Professional Development (CPD).
* Proven experience providing housing support to individuals with complex needs, including those leaving institutions such as prison or hospitals.
* Strong understanding of housing legislation, safeguarding, and social care policies.

* Excellent organizational and time management skills to handle a flexible and complex caseload.
* Strong communication skills to engage with diverse client groups and professional stakeholders.
* Ability to assess, plan, and implement Sustainable Housing Support Plans.
