The post holder will be a crucial part of the Hospital Discharge Team, managing and supporting health care clinicians in complex discharge planning. This post has a particular focus on fostering good relationships with care home providers and supporting good transitions from hospital to care home environments.

As a key member of the Royal Devon University Healthcare NHS Foundation Trust, you will be joining an exceptional team working together to help our patients to stay healthy when they need it most.
Job responsibilities
The Hospital Discharge Team has an exciting fixed term or secondment opportunity for an enthusiastic and motivated registered nurse or occupational therapist, to assist with managing and supporting complex discharges. This role involves using a patient-centric approach to completing nursing assessments and formulating discharge plans.
The hours needed for the position are 37.5 hours per week between the times of 8am - 6pm. You will form part of a rolling staffing rota, covering the service 7 days a week and flexibility in your working is appreciated. You will be required to work one weekend in every four to support the team.
The successful candidate will have the ability to work as a confident autonomous practitioner. You will need high level clinical assessment and problem-solving skills to manage complex patients who require assessment of needs prior to discharge.
This post allows you to draw on skills gained from both acute and community experience, due to the broad range of patient presentations and work both over the phone and face to face in the community and acute hospitals.
You will also work with Specialist Nurses and Physiotherapists, enabling you to expand your knowledge in multidisciplinary complex care. You will need to demonstrate flexibility, good time management skills and excellent communication and interpersonal skills.

Working Pattern: 37.5 hours per week between the hours of 8am and 6pm. Monday to Friday.

This is a fixed term contract for 12 months. For current NHS employees, this post could be a secondment opportunity.
