Job responsibilities The implementation of Health Promotion and disease management strategies (in line with the National Service Frameworks) and provision of a full range of nursing services to the patients of the practice .Chronic Disease Management as required by the practice.Accountable to Clinical Partners, Managerial to Practice Manager. The completion of health checks, health screening, immunisation and vaccination programmes.To provide a service to patients with injuries or conditions requiring a dressing.
