We are looking for a social worker with experience of working in Adult Social Care, specifically within a hospital discharge team.
The social worker should have a good understanding of the social worker’s processes including assessment, intervention, and review, as well as the key legislation that underpins these processes.
Responsibilities include:
1. Undertaking assessments of service users and carer needs using a person-centred approach in accordance with agreed statutory and directorate procedures.
2. Assessing capacity and coordinating best interest meetings and decision-making for those who lack the capacity to make decisions regarding their discharge destination.
3. Assessing and managing risks, ensuring any safeguarding concerns are addressed in a timely manner.
4. Applying a strengths-based approach in practice.
5. Using management information systems to find, analyze, and update information, ensuring compliance with service procedures and data protection legislation.
6. Devising and implementing person-centred support plans, negotiating a range of appropriate services and interventions to meet assessed needs and risks.
7. Monitoring and contributing to the review of plans relating to the assessed needs of service users, their families, and carers in accordance with directorate procedures.
8. Using a safeguarding framework to investigate and manage risks where individuals have experienced or are likely to experience significant harm.
9. Providing concise written and verbal reports that can be used within the legal context.
10. Identifying and implementing strategies for responding appropriately to concerns about practice or procedures, seeking guidance if required.
11. Convening, chairing, and participating in meetings/forums in line with procedures and developing effective working relationships to foster holistic and integrated practices.
