We have an awesome vacancy for you Hours: 21 per week Location : Darlington (with some elements of homeworking) Contract type : Permanent Interview date : w/c 13th January The service we provide is unique to each person that uses it as it is based upon personal circumstances and support needs. As a Dementia Adviser you will offer support to your clients in a variety of ways, whether in the client's own home, or at other locations in the community, face to face, by phone, letter, or email. You will also assist service users to access other services, providing signposting and referrals. About the role Supporting people affected by dementia, families and carers to access vital services. Improving people's sense of well-being, enabling them to have more control over their lives. Assisting with the identification of their needs, providing information and access to relevant services. Promoting our amazing Dementia Advice service and building relationships with a range of health and social care professionals. About you Ability and interest in supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives. Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels. Able to network effectively and collaborating with other professionals to achieve a positive outcome for people. Good IT skills, experience working with client or patient databases and virtual meeting software (Teams / Zoom). The ability to travel across Darlington independently when required.
