An exciting opportunity has arisen within our Families, Young People, Children's and Learning Disability (FYPCLDA) Directorate for a clinical team leader (CTL) post in the 5 -11 school nursing Leicestershire, Melton and Rutland team. This post will be a seconded post to enable someone to develop their knowledge into the CTL role. We are looking for a highly motivated, compassionate, and skilled Specialist Community Public Health Nurse in school nursing, who will bring energy for transformation to the delivery of service for children and young people across LLR. The post holder will provide operational line management for the school nursing team covering Leicestershire and Rutland, who will support the school nurse public health agenda with clinical contact dependent on hours. The CTL will also support the family service managers to operationalise and embed service improvement where required. Car owner/driver and current driving license essential. Main duties of the job The successful candidate will perform and monitor comprehensive assessments of clients/service users needs. Plan, implement and evaluate care delivery according to changing health care needs. Takingcontinuous management responsibility for the delivery of care needs of individual and groups of clients/service users. The post holder will ensure patient information is collated, monitored and fully accurate, maintaining complete and contemporaneous records in line with NMC/ professional bodies and Trust standards. Promote the involvement and engagement of clients/service users and carers / relatives in the assessment, planning, delivery and evaluation of care. Work collaboratively and ensure maintenance of effective multi-disciplinary communication and with other professionals, agencies and stakeholders to ensure needs are met in relation to care input, support for ongoing care needs and safe discharge.
