An opportunity has arisen for a part-time clinical pharmacist (specialised interest in Diabetes) to join South Oxhey Surgery. The post holder is a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists/clinicians in general practice.
Main duties of the job
* Patient facing long-term condition clinics to review the ongoing need for each medication.
* Patient facing / Telephone clinical medication reviews.
* Patient facing / telephone medicines support.
* Unplanned hospital admissions review of medications and reconciling of medications.
* Identification of high risk patients.
* Manage repeat prescription requests, resolving queries where possible within scope of practice. Ensure patients have appropriate monitoring tests in place when required.
* Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance.
* Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
* Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
* Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary.

The South Oxhey Surgery is located within walking distance from Carpenders Park train station. The practice looks after 4000 patients and uses the EMIS clinical system. We have a friendly and supportive team, and the surgery offers onsite car parking.
Job responsibilities
The post holder will be a pharmacist with a specialised interest in Diabetes, which includes a patient-facing role. The post holder will take responsibility for areas of chronic disease management including clinical medication reviews and proactively managing patients with polypharmacy.
Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
Provide support to general practice staff regarding prescription and medication queries. Help support the practice's repeat prescription systems, management of acute prescription requests, and medicines reconciliation on transfer of care from acute and community hospitals.
Support practice systems for safer prescribing, providing expertise in clinical medicines advice.
Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
Provide clinical support on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes quality improvement framework, enhanced services, and contribute to the delivery of the local medicines optimisation strategy.
Help manage practice workloads by ensuring that the addition of the pharmacist to the clinical team results in a reduction in workload for medicines optimisation and related processes for other clinicians and non-clinicians.
Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary.
Maintain own clinical and professional competence and be responsible for own continuous professional development.
Ensure full compliance with Care Quality Commission standards for safe and effective care.
Person Specification
Qualifications
* As a minimum, you will hold a degree in Pharmacy (Masters or equivalent).
* Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society.
* Ideally, you will be a non-medical independent prescriber.
Experience
* Excellent clinical and medication knowledge.
* Understanding and experience of working as a pharmacist in a GP practice setting, with a confident approach.
* Experience of undertaking medication reviews.
* Commitment to continue to learn and develop.
* Ability to be adaptable and flexible and be a proactive problem solver.
* Excellent interpersonal patient-facing communication skills.
* You should be IT literate and confident in using practice systems.
