Job Description
The main purpose of the Care Coordinator is to be responsible for all tasks related to arranging client scheduling. This includes matching and scheduling care professionals to clients.
This role would suit an individual who enjoys solving puzzles, arranging logistical solutions, and dealing with problem-solving.
As the Care Coordinator, you will:
* Assist with the onboarding of new joiners.
* Coordinate the forward planning of care schedules and ensure it is consistently achieved in order to provide continuity of care to our clients.
* Support the Registered Care Manager and the Care Supervisors to ensure that any communication with external sources such as next of kin, GPs, nurses, physiotherapists, and any other professionals involved is prioritized if they impact on the care and wellbeing of the client.
* Ensure that all Care Professional holiday, sickness, unplanned absences, and all emergency calls are documented electronically.
* Cover care calls and gaps in the care schedules where they arise and cannot be covered.
* Be part of the care team on-call rota system during evenings and weekends as and when required.
* Understand the arrangements for ensuring all clients are safeguarded against the risk of abuse and respond appropriately to any allegations of abuse or complaints regarding service delivery whilst complying with Home Instead policies and procedures.
Qualifications
Our ideal candidate will:
* Have a high level of attention to detail gained in a previous customer service-oriented role.
* Require a strong knowledge of IT systems.
* Be self-motivated and demonstrate integrity, trustworthiness, and reliability.
* Have the ability to build and maintain strong working relationships with clients, care professionals, and colleagues alike.
* Have previous experience working within care, which is preferable but not essential.
* Have experience of coordinating rotas or schedules, which is also desirable but not essential.
* Hold a valid driving licence and have use of your own vehicle.
