Senior Practitioner – Integrated Triage & Assessment Services
This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2025.
The successful candidate may be eligible for up to a maximum of £8,000 (including VAT) towards the cost of expenses incurred due to relocating to Ceredigion. The relocation scheme is only available to those with a legal right to work in the UK. We do not currently offer sponsorship.
We are currently looking to recruit a permanent Senior Practitioner Social Worker to join the Integrated Triage and Assessment team, Porth Gofal Service.
This post is an exciting opportunity for a dynamic and committed member of staff to work in a multi-disciplinary environment, supporting vulnerable individuals and their families within hospital and community settings.
Porth Gofal is the integrated decision-making hub for Ceredigion, which triages and determines the level of need of all cases and the best outcomes for those individuals.
Porth Gofal is the heart of service provision that focuses on ensuring that citizens receive the intervention that is best placed to meet their needs, or, where necessary, to guide them to early help or specialist services.
What Porth Gofal provides:
* A responsive service to citizens living in Ceredigion.
* Applies a Strength-based needs assessment focusing on service user choice and control using the Signs of Safety framework.
* A reduction in the time Social Care professionals spend determining eligibility for statutory services.
* A legislatively compliant service that meets the statutory requirements of the SSWBA.
* A safe service that identifies vulnerable and at-risk citizens and responds effectively.
What Porth Gofal achieves:
* Provides information, advice, and assistance about community health and social care services for adults that improves the connection between people and their communities.
* Encourages a balanced approach to the assessment of need ensuring that citizens are at the centre of conversations, decisions, and the “what matters” conversation takes place.
* Multidisciplinary triaging of referrals to identify the right professional to respond (e.g., physiotherapist, Occupational Therapist, or Social Care workers) to ensure the right response at the right time.
* Provides more consistent, coordinated assessment for care and support.
* Improves the flow of information between services, resulting in faster decisions being made about people’s needs and support.
* Reduces hospital admissions by providing rapid wrap-around reablement response service.
* Provides an in-reach service to reduce delayed transfers of care by providing timely discharges from hospital.
A commitment to integrated working with a focus on positive rehabilitation outcomes for citizens is essential.
The ability to travel throughout the county is essential.
Must have two years post-qualifying experience with experience of supervising staff.
A bilingual policy is in operation, and candidates who do not have the linguistic ability in Welsh will be supported and encouraged to achieve the ALTE standard within two years.
