The Vacancy North Lincolnshire Home First Community support service is a registered Rehabilitation and Reablement service that provides short term intervention services, including therapy where identified, to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to ensure that a person can remain at home and feel confident and is safe to live as independently as possible. Rehabilitation Assistant post holders provide rehabilitation support to individuals following accident or illness with the aim of returning to independence. This involves following therapy plans and working in conjunction with other health and social care professionals. The role also involves direct work such as supporting with moving and handling, attending to personal care needs and administering medication as well as maximising opportunities for social inclusion and emotional wellbeing. The service operates with AM shifts 07:00 to 13:35, and PM shift 15:55- 22:30 and 'mid' duty shifts which vary between 07:00-23:00. In addition, there are enhancements payable for working weekends and bank holidays as well as a percentage in lieu of annual leave as a casual worker. As the Home First Community support team provides service to people in their own homes so the ability to drive and transport across North Lincolnshire is essential. As a Casual Rehabilitation Worker you will be part of a fast paced and developing service that has opportunities for further development and progression. A previous qualification in care is desirable, though not essential, as full induction/training will be provided.
