The Out Of Borough Hospital Discharge Team is a dynamic and vibrant service for people living in Croydon. The Out of Borough Hospital Discharge Service leads on the discharges of Croydon residents with care and support needs from a range of Out of Borough Hospitals. The team works in partnership with NHS partners to ensure timely and safe discharges from Hospital. For this role, we need a committed knowledgeable social worker to help us with our core business – empowering and supporting Croydon residents who need signposting, community support, or even some direct help from the Council. We have a strong operating model of strength-based practice. Experienced Social Worker The successful candidate should be a registered Social Worker with proven extensive experience in hospital discharge within Adult Social Care. You will have had years learning your role, be experienced at providing supervision to social workers, managing a range of complex cases for adults, supporting them and their families throughout their transition from hospital into the community. We work with adults who have a wide range of complex health and social issues. We challenge inequality and traditional responses to service delivery and ensure that we employ a strength-based approach to our work. As an Experienced Social Worker, your responsibilities will include strong partnership working with all the key partners, these include health, social care, voluntary organisation etc. You will be required to develop strong partnership working through the Alliance with the Out of Borough Hospitals, Integrated Care Board, Public Health England; to enable the Council to achieve its corporate objectives of providing integrated whole family preventative support. You will also work with all services to improve the resident experience through innovative and creative service improvements. You will be responsible for ensuring that the work of the service complies with all relevant legislation and statutory guidance including, but not limited to the Care Act 2014, the NHS Act 2006, the Mental Health Act 1983 (as amended in 2007), the Mental Capacity Act 2005, Deprivation of Liberty and the transformation agendas for health and social care.
