We are looking to recruit a part-time, fully qualified, experienced Advanced Clinical Practitioner to join our team.
The vacancy is for a part-time ACP, with weekly contracted hours between 28 to 34 hpw, ideally Monday, Tuesday, Thursday afternoon & Friday's; however, this is negotiable.
Main Duties of the Job
The Advanced Clinical Practitioner will act autonomously within their professional scope of practice, providing care for patients through face-to-face consultations, telephone consultations, and potential home visits. The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers, and colleagues, ensuring clinical safety and excellence. The Advanced Clinical Practitioner will need to prioritize and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. The post holder will use skills, knowledge, and competencies as a qualified Advanced Clinical Practitioner to be responsible and accountable for managing patient caseloads for treatments, referrals, admissions, and discharging patients as appropriate.

Our team comprises 3 GP Partners, 1 part-time salaried GP, 1 Advanced Nurse Practitioner, 3 Practice Nurses, 1 Assistant Practitioner, 1 Healthcare Assistant, Practice Manager, Assistant Practice Manager, supported by an organized and well-structured administrative & reception team. PCN additional roles within the practice include MSK practitioner, Mental Health Practitioner, Clinical Pharmacist, Advanced Clinical Practitioner, Paramedic, and Social Prescribers. We are a well-organized, structured practice, dedicated to providing excellent clinical care. Our teams throughout the practice are friendly and supportive.

Job Responsibilities
1. Work as part of a multi-disciplinary team to deliver safe and high-quality care.
2. Evaluate and prioritize patient symptoms and provide general advice and reassurance in line with guidelines.
3. Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
4. Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
5. Prescribing and reviewing medication that is appropriate to patient needs and in accordance with the latest evidence-based practice, national and practice protocols, and within the scope of qualified practice.
6. Diagnose and manage acute and chronic conditions.
7. Request diagnostic investigations and interpret within agreed protocols e.g., x-ray, blood tests, scans, etc.
8. Assess, diagnose, and treat patients who require medical attention and/or refer to appropriate agency.
9. Refer patients to other members of the multi-disciplinary practice teams, secondary care organizations, and other organizations as necessary.
