Job summary To assess, implement and monitor the care needs of patients safely within their own homes, using technology to view observations and conducting telehealth consultations without direct supervision. To work alongside the multi-disciplinary team regularly to provide the holistic care to patients on the virtual ward. The post holder will manage their workload and escalate any concerns to their senior nurse and/or consultant on call.
