Ward 22 is currently looking to recruit a proactive and patient centred Discharge Coordinator. As a ward, we pride ourselves on our effective teamwork. As a Discharge Coordinator on Ward 22, you will be required to interact closely with all members of the Multi-Disciplinary Team and other external health and social care services in order to facilitate safe and effective discharges for our respiratory patients.
Base Location: Royal Bournemouth Hospital

Person Specification
Qualifications
* NVQ level 3: health/social care management or equivalent
Experience
* Experience of working in a hospital setting
* Understanding of complex discharge planning in care of the elderly, stroke, long-term conditions, and vulnerable adults
* Experience of supporting service users' specialized care needs to enable informed decision-making
* Use of IT including Windows, Word, Excel, and PowerPoint
* Assessment, planning, and organizational skills
Knowledge
* Understanding of NSF's, CHC Framework, Delayed Discharge Act, and Reimbursement
Disclosure and Barring Service Check
