Main duties of the job
As a Health Coach, you will work within our Wiltshire Health Improvement Hub to deliver smoking cessation programmes and one-to-one coaching to Wiltshire residents. Your role also involves working with colleagues across the Council and with community partners, contributing to broader projects enhancing the health and wellbeing of Wiltshire residents. This role requires availability to work outside of standard business hours, including evenings and weekends. Travel across Wiltshire is required; therefore, access to your own vehicle is essential.
We are looking for an innovative and motivational individual with community engagement experience and the ability to deliver initiatives for positive behaviour change. Ideally, you will hold at least a Level 3 Health Training/Coaching qualification or equivalent, along with a strong knowledge of health improvement and behaviour change interventions. Experience reading and using medical or surgical terms would be desirable but is not essential.

Job Description
Job Responsibilities
The Health Improvement Hub places Health Improvement Coaches to work in local communities to identify and support those at greater risk of poor health. They work on both a one-on-one basis and facilitate group sessions, supporting people to make healthy lifestyle changes and improve their lifestyles to enhance both mental and physical wellbeing. Health Improvement Coaches are behaviour change specialists and Stop Smoking Practitioners, capable of offering health assessments including blood pressure readings. They work in partnership with local clinical and non-clinical services within the NHS and other organisations in the public, private, and voluntary/community sectors.
Specific Duties and Responsibilities Include:
1. Develop and deliver evidence-based behaviour change group programmes to target groups across Wiltshire.
2. Work on a one-to-one basis with individuals under a lone-working policy for a period of six sessions per individual or through a more flexible approach as agreed by the client and their Health Improvement Coach, using behaviour change techniques to build confidence and motivation and to overcome barriers to change.
3. Undertake follow-up maintenance assessments of clients at 3, 6, and 12 months.
4. Conduct a 30-minute pre-consultation appointment for mental wellbeing clients, assessing readiness for change, eligibility for the service, and client expectations.
5. Help individuals develop, implement, and review an action plan to support behaviour change and sustain those behaviours by setting SMART goals.
6. Assist individuals in accessing local community services as part of their self-care and self-management.
7. Maintain up-to-date information regarding local groups and activities, ensuring recommendations are for recognized and regulated groups.
8. Ensure all government guidelines and information given to clients is accurate and up to date.
9. Target client work appropriately using knowledge of local health deprivation and inequalities.
10. Engage in community-based work to generate referrals and convey key health messages.
11. Use judgement to ensure safeguarding of adults and children, seeking immediate medical/professional support when necessary.
12. Plan sessions creatively using Wiltshire Council resources to meet guidance/standards while maintaining interest.
13. Conduct health assessments ensuring all health and safety requirements are met.
14. Work as a level 2 stop smoking advisor, delivering interventions to support quitters.
15. Provide evidence-based preventative mental health and wellbeing support to clients.
16. Provide evidence-based physical activity support to clients seeking to increase their physical activity.
17. Provide evidence-based alcohol reduction support to clients seeking to reduce their alcohol intake.
18. Signpost referrals to suitable services when appropriate.
19. Risk assess private room spaces for client meetings.
20. Follow the lone working policy, ensuring health and safety guidelines are met.
21. Work with partners to promote the Health Improvement Coach service.
22. Maintain accurate written client records and transcribe them onto the national Health Improvement Coach database following data protection guidelines.
23. Monitor progress of individuals and determine the need for alternative support.
24. Report concerns about individuals to managers as appropriate.
25. Support public health at health fairs and events.
26. Ensure equipment used as part of the service is functioning effectively and undertake quality assurance procedures.
27. Deliver health and wellbeing workshops and training to community groups.
28. Build ongoing professional relationships with a range of professionals within the health and social care system.
Person Specification
Qualifications
Essential
* A passion for health and wellbeing and a desire to support individuals across Wiltshire.
* Experience of delivering evidence-based behaviour change programmes/interventions.
* Hold a Level 3 Health Trainer certificate or equivalent.
* Experience as a Stop Smoking Practitioner.
* Knowledge of factors supporting good physical and mental health within the local community.
* Ability to apply new evidence and knowledge into practice.
* Proven experience in health/social care or customer service roles.
* Experience working with local community groups.
* Experience supporting the health and wellbeing of adults.
* Experience in delivering and facilitating group work in behaviour change.
* Knowledge of local population health needs.
* Knowledge of local services and how to support individuals in accessing them.
* Specialized knowledge of health inequalities and behaviour change methods.
* Excellent verbal communication skills.
* Strong active listening skills.
