We have a vacancy for an additional fixed term Advanced Clinical Nurse Specialist (Band 7) to join the Marie Curie REACT project based in Bradford. Our team includes nurses, physiotherapists, and doctors working in partnership with other services to prevent unnecessary hospital admissions for patients with unmet or rapidly escalating palliative care needs. Patients will be identified in the Emergency Department of Bradford Royal Infirmary or by community colleagues as having unmet symptom control/end of life care needs that can be safely managed at home with additional short-term urgent support from our REACT community team.
This is a career-enhancing opportunity to improve patient and family experiences and reduce inequalities in access to specialist palliative care across Bradford.
You will provide specialist palliative care nursing services to people with terminal illnesses, their families, and health professionals. Responsibilities include working autonomously as an expert practitioner in palliative care, practising as a Non-Medical Prescriber, managing a caseload of complex patients, and supporting the multidisciplinary team.
As part of the leadership team, you will foster a compassionate, person-centred environment, lead by example, and demonstrate advanced clinical knowledge in palliative care. You will also provide evidence-based education and undertake quality improvement initiatives.
