The successful candidate will work in partnership with Cambridgeshire and Peterborough Integrated Care Board (ICB) and determine individuals eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process. The role involves completing Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best Interest Decisions (where applicable) identifying individuals' needs and supporting the Multidisciplinary team's recommendation on eligibility for NHS Continuing Healthcare. Where individuals are not eligible for NHS Continuing Healthcare, but are eligible for care and support from Cambridgeshire County Council in line with the Care Act 2014, the successful candidate will implement this. The successful candidate will support Cambridgeshire County Councils Adult Social Care workforce to improve their confidence, skills, and knowledge in relation to NHS Continuing Healthcare.This will be achieved by providingtraining, coaching, mentoring and advice to staff in relation to the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022).
