Job overview This is an exciting opportunity to work as part of the existing Health and Justice Reconnect Service in Sussex. This role will include: • Independently undertake face to face assessments to support the identification of multiple vulnerabilities both within Health and Justice system including prison and the community. • Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service. • Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support. • Liaise with other agencies as required dependant on the person’s presentation, work closely with other agencies. • Due to the nature of the role a driving licence and use of a car will be required as part of the role. • The main base for this post will be in New Haven however attendance at HMP Lewes, HMP Ford and Immigration Removal Centre (IRC) Gatwick will be required – due to this additional vetting clearance will be requested and Enhanced level 1 vetting clearance will be required. • Usual hours of work will be 08:30-16:30 Monday to Friday. Main duties of the job: • Independently undertake face to face and electronic assessments to support the identification of multiple vulnerabilities of people who are in prison or have been released from prison. • Within prison and IRC the assessments will take place up to 12 weeks before the person’s release and in the community this will include assessments of people who have been released from prison in the past 28 days. •The candidate must be able to travel to locations across Sussex and therefore will need to have use of a car with business insurance due to the distance and time required - service users may need transporting as part of the Reconnect release plan. • In addition cover to the wider Health & Justice service – Police Custody, Court, Youth Team and the Outreach team for Liaison and Diversion may be required. • Work within limitations and escalate to Practitioner as appropriate. • To support Practitioners, STR Workers and Lived Experience workers and Admin as necessary. • Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service. • Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support. • Liaise with other agencies as required dependant on the person’s presentation, work closely with other agencies, such prison staff & Health Care Professionals, and community agencies.
