Play an active role in the design and implementation of a holistic diabetes service. Put the patient at the center of the diabetes service. Take lead clinical responsibility for crafting treatment plans. Review diabetic blood test results. Provide treatment plans based on current best evidence. Screen incoming letters from secondary care and action recommendations Encourage patients to take ownership of their condition. Co-ordinate the provision of care to nursing home patients. Co-ordinate the provision of care to house bound patients. Provide a supporting role to the research lead GP for research relating to diabetic care. Provide clinical supervision for clinicians working in the diabetes team (Nurses, HCAs etc). Work to maximise referrals to the NDPP.
