We are seeking a motivated and experienced Pharmacist to join our Clinical Pharmacy Team based at Heartlands Hospital site providing a clinical service to the Severe Asthma speciality. This is a year rotation into Severe Asthma to support the service. After the 12 months you will embark on our senior support pharmacist rotational program. The successful applicant will become part of a large clinical pharmacy team. They will work closely with the Specialist Pharmacist for Respiratory Medicine, medical practitioners and specialist nursing team members. Through managing and supporting Clinical Support Pharmacists and clinical pharmacy technicians (CPTs) you will work to support and improve patient stay in hospital whilst retaining direct patient contact. You will need an ability to develop effective working relationships, to challenge and to prioritise your own work as well as the work of the team. Main duties of the job You will aim to reduce referrals and attendance at the Emergency Department; support proactive safe and timely discharge of complex inpatients to community care; and deliver care based around individual patient needs. Key duties Include: Be an integral member of the multidisciplinary virtual ward team. To provide clinical pharmacy service to virtual ward patients to either avoid hospital admission, or to support with an early supported discharge from hospital. Arrange communication aids such as verbal handover/ written documentation on System One or EMIS. Prioritise all incoming virtual ward and pharmacy work on a daily basis, including responding quickly and appropriately to urgent queries, referring to the team where appropriate. Attend multidisciplinary meetings as stipulated by the virtual ward and pharmacy team regularly to discuss workflow. When suitably qualified, to prescribe medicines for agreed patients within their capacity of an Independent Prescriber. Produce a summary of key achievements and audits completed/pending on a quarterly basis. To understand and deliver Cost Improvement Programs (CIP) to support the speciality and department. In addition completion of audits and Quality Improvement Project (QIP) to present to the speciality meetings will be supported. You will join a team of clinical pharmacists who provide the on-call service with a back-up provided by the Specialist and Advanced Clinical Pharmacists.
