Job Duties
The Western Tidewater Health District is seeking a highly motivated LPN to join a team of professionals committed to providing public health services at the Franklin Health Department. This position serves as the Medication Assistance Program Coordinator. The MAP Coordinator provides assistance to medically indigent individuals in obtaining prescription and non-prescription items recommended by their Physician that will benefit their health status but is not covered by their insurance as well as providing community outreach to promote community awareness of the program. This position is responsible for gathering and analyzing data in support of the program as well as communicating program reports to the Franklin/Southampton Charities who provided the grant funding.
This position will function as a Licensed Practical Nurse in assigned area(s):
1. Interview clients, provide counseling according to applicable program policies, procedures, and health guidelines.
2. Apply critical thinking and cultural awareness to all communication modes.
3. Complete required documentation in client chart.
4. Make appropriate referrals for medical care and community resources.
5. Provide administrative assistance as needed to support HD activities and programs.
6. May perform other duties as assigned.
7. May be required to assist in the event of an emergency declaration.
The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The Western Tidewater Department of Health services the City of Franklin, the City of Suffolk, Isle of Wight County and Southampton County.
This is a RESTRICTED position, which is solely funded by the MAP Grant for a year period ending in December. The availability of funding is scheduled for review annually and periodically thereafter. Continued employment is contingent on the continued availability of funds.
VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year's seasonal influenza vaccination.

Minimum Qualifications
8. Licensure as Practical Nurse required by state code.
9. Knowledge of techniques and practices of the nursing profession, of public health practices, of quality assurance concepts, of teaching principles, of principles of decision making and problem solving techniques, of accurate record keeping and quality control, of socio-economic conditions relating to personal needs of clients
10. Skills in performing technical procedures, in communication orally and written, in interacting with clients and the general public
11. Ability to document clearly and accurately utilizing the health department’s accepted form of documentation
12. Ability to establish and maintain working relationships with individuals and families to function as a team member
Additional Considerations
Current CPR certification
Prior experience in a public health setting preferred
