You will monitor patients and evaluate care as part of the role, working collaboratively with other teams within the hospice, and the wider healthcare community, to ensure that we are responsive as their needs change. You will work closely with the Community Services Manager and will have the opportunity to contribute to the development of the evolving service, including new programmes, workshops and support groups. You will facilitate group sessions, with the support of the multidisciplinary team, managing and encouraging group dynamics, providing support, and using excellent communication skills to enable Advanced Care Planning and End of Life Care discussions. You will be required to use your nursing experience to provide the highest standards of evidence based care.
