Responsible for providing specialist evidence-based advice to all disciplines of staff, patients and carers relating to Ophthalmic conditions Provides specialist advice relating to individual patients and groups of patients in relation to Ophthalmic conditions Provides an efficient service and utilises full potential of their nurse led clinics. Contributes to the development of plans of care for patients who need input from the Ophthalmology team. Participates in the development of policies and guidelines relating to Ophthalmology, which may have impact in a variety of areas throughout the Trust. Uses audit and observation to ensure that other staff follow relevant policies. Has responsibility with other members of the team for ensuring that training is provided on Ophthalmic issues to all staff from a variety of professions. Participates in the evaluation of new equipment and services in relation to Ophthalmic conditions. Run Nurse Led Clinics to provide highly skilled care on behalf of the Ophthalmology Consultants for patients with Ophthalmic conditions g and provide specialist nursing advice for patients. Ensure a quality patient centred approach to care, which recognises the needs and views of patients and carers. Responsible for recording patient information. Both written and using computer software as appropriate. Only reveals sensitive information to relevant individuals from this data storage. Regularly participates in audit, research and surveillance. Contributes to initiation of these programmes related to Ophthalmology using a recognised audit methodology. Responsible for maintaining, developing and completing own personal portfolio and submitting evidence for revalidation as required. Responsible for maintaining mandatory training compliance. Responsible for reporting adverse incidents or errors where staff or patients may be at risk. Participates in and proposes modernisation, improvements and service development within the team. To undertake any other duties which may be reasonably regarded as within the nature of the duties, responsibilities and grade of the post as defined, subject to the proviso that appropriate training is given. Provide education and support to the patients, carers and staff.
