Role Overview With an ageing population and pressure on acute and community health and social care, enabling people to stay at home and live independently is a key healthcare challenge. The Discharge 2 Assess Pathway 1 Service ensures that individuals in Lincolnshire hospitals capable of returning home do so, providing short-term assessment, recovery, and rehabilitation. This service follows the 'Discharge to Assess' (D2A) principles where assessments for long-term needs occur at a person's home. The roles are available across Lincolnshire, with service hours from 0800 to 2000, 7 days a week, and part-time options considered. Responsibilities Provide rehabilitation and reablement to patients post-hospital discharge. Promote independence and contribute to assessments determining future needs. Work closely with multi-disciplinary teams to promote independence and prevent dependence. Undertake environmental assessments within patients' homes independently. Communicate effectively with patients, carers, health professionals, and outside agencies. Participate in the delivery of complex care as prescribed by qualified practitioners. Plan and progress simple patient interventions with minimal supervision. Ensure the safe use and ongoing monitoring of equipment. Identify needs and advise on equipment and resources, requisitioning when authorized.
