Job overview
The Universal Crisis Team for Newcastle and Gateshead currently has vacancies for Full Time Community Practitioners to work within the Working Age Adults pathway. We are looking to recruit motivated and committed individuals into the team.
The post holder will be part of a team working under the crisis model, offering support within the WAA pathway. The successful candidate must have a track record of clinical experience in working with risk and complexity.
Main duties of the job
1. To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
2. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies, and the public.
3. In collaboration with the Service User and Carer, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews.
4. Consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
5. Deliver evidence-based Clinical and Psychologically based Interventions.
6. Work as a member of the Multi-disciplinary Team ensuring high quality individualised care.
7. Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.
8. Provide compassionate care based on empathy, kindness, respect, and dignity.
9. Incorporate a client-centred philosophy into nursing practice and collaborative care.

Person specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent.
* Evidence of post-registration training in clinical/psychological interventions relevant to the post.
* Proven track record of post-registration clinical experience.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA, and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Post Registration Qualifications in Psychological Interventions.
* Experience of working within a community team.
* Non-Medical Prescribing.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Understanding of difficult to manage/challenging behaviours.
Skills and Competencies
* Proven ability to complete comprehensive assessment of complex mental health needs.
* Identify treatment options as per NICE Guidelines.
* High standard of written and verbal communication.
* Ability to work under pressure.
* Clinical Leadership.
