Advanced Clinical Practitioner (ACP) - General Practice

An opportunity has arisen for an ANP/ACP to join our friendly and supportive General Practice team.
We are looking for a self-motivated, flexible individual, with good organisational and communication skills who is able to work effectively within our clinical team to undertake patient assessment, diagnosis and clinical management.
You will need to have the ability to work in ever-changing, demanding situations. You will work autonomously within your professional scope of practice adhering to the four pillars of advanced practice: clinical practice, leadership and management, education and facilitation of learning, and evidence, research and development.
You will need to demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
You will need to be able to work collaboratively within the practice team to meet the needs of patients and support the delivery of policies and procedures.
Main duties of the job
The main duties involve:
1. Assess/examine patients, diagnose, plan, treat and review/evaluate.
2. Assessment may be face-to-face by appointment or by telephone as part of a triage/unscheduled care team and will include patients presenting with an undifferentiated diagnosis.
3. Diagnose and manage patients presenting with both acute and chronic conditions based on a management plan which includes a medication plan and other interventions.
4. Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
5. Assess and manage patients' physiological and psychological needs, including patients with complex needs, multiple diagnoses, and polypharmacy.
6. Identify patients at risk of developing a chronic illness and develop a plan for minimising the risk of worsening.
7. Advise and educate patients, promoting understanding, self-management, healthy lifestyle choices, and sources of support and compliance with a treatment plan and medication.
8. Other responsibilities include ensuring all clinically generated administration is completed in a timely manner.

We are a small Practice with approx. 6,000 patients.
The practice is part of Derbyshire Community Health Service NHS FT with a Practice Manager, Clinical Lead and established clinical and admin teams to ensure the day-to-day smooth running of the practice.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

Experience
* Clinical experience at senior level
* Management of minor illness, minor ailments, and injuries
* Clinical Leadership
Qualifications
* Professional registration with NMC or HCPC
* MSc in Advanced Clinical Practice (or equivalent)
Knowledge and Skills
* Advanced clinical practice skills including management of patients with long-term conditions
* Management of patients with complex needs and chronic diseases
* Practical understanding of primary care and community services
* Implementation of best practice
