The purpose of the roleis to provide specialist dementia nursing expertise and support to thoseaffected by dementia across RAVEN PCN, with a focus on family carers/supporters who experience difficulties.
Main duties of the job
To provide specialistnursing assessment, evidenced based intervention, practical advice andemotional support for family carers of people with dementia and whereappropriate people with dementia themselves, at all stages includingbereavement.
To offer expertise andguidance to professional colleagues in supporting family and relationshipcentered approaches and best practice in dementia care.
To assist with thedevelopment, evaluation and audit of this specialist area of practice with thePCN.
To provide up-to-dateclinical advice, support, liaison, and in negotiated circumstances educationand training to other staff.
To refer to theAdmiral Nurse competency framework to support the development of own practice.
To provide clinical supervision to others innegotiated circumstances
To support co-ordination of care needs forindividuals diagnosed with dementia and cognitive impairment.
To support from the point of diagnosisthroughout the journey, ensuring patients are treated to evidence-basedstandards in accordance with agreed Care Plans.
To provide a clear and accessible point ofcontact for guidance, support and navigation, working closely with other healthand social care organisations and voluntary agencies.

Job responsibilities
Main Duties
1. Clinical
To meet the multiple and often complex needs of families affectedby dementia through ongoing assessment of health and wellbeing, including risk assessment,developing, implementing and evaluating intervention plans.
To advise on, recommend and provide a range of effective evidence-basedinterventions at all stages of the familys experience of dementia, paying particularattention to loss, transitions and changing relationships.
To assist clients families affected by dementia and orcolleagues, to develop skills and competencies in understanding and coping withchallenges and difficulties they may face, in both individual and groupsituations. For example, managing complex conditions, adapting responses whensupporting the person with dementia, changes in relationships, distressedbehaviour.
To provide a range of psychological and social interventions topromote health and assist with the alleviation, prevention and management ofstress, depression, anxiety and other mental health needs of carers
To maintain clinical records using local information systems andprovide verbal and written feedback appropriately and as required, to carers,persons with dementia (as appropriate), referring agencies and professionalcolleagues.
To liaise with GPs, case managers, primary health carepractitioners, statutory and voluntary agencies regarding the best possiblecare for the family affected by dementia.
To promote the health and wellbeing of families affected bydementia in line with health and social care policy, guidelines andlegislation.
To act as advocate for family carers (with permission) tofacilitate access to services and influence care provision
To identify development options for addressing inequality and tocontribute to strategies designed to improve access to services for people withdementia and carers who are socially excluded.
To ensure that care is delivered in accordance with nation and localstandards such as those recommended in NICE guidelines, national dementia and carer strategies/ plansand all relevant Raven PCN policies.
To recognise the limits of own competency and professionalboundaries and to make appropriate and timely referral in respect of clientsneeds
To facilitate and participate in case conference meetings, networkmeetings and similar, giving feedback and liaising with all concerned.
To provide consultancy and guidance to those working in dementiacare and to work collaboratively with health/social services and the voluntaryand independent sector (as appropriate) to support best practice in dementiacare
To promote the rights, interests, needs and choices of carers andpeople with dementia in the planning, delivery and evaluation of care andservices.
To facilitate consultation with carers about care and servicedelivery; to evaluate and provide feedback.
2. Professional
To deliver nursing practice in accordance with the agreed policiesand procedures of RAVEN PCN,the NMC Code of conduct and other relevant nursing policy.
To development/training days as agreed with RAVEN PCN.
To maintain professional registration in line with NMC guidanceincluding re validation.
To participate in practice-based evaluation, audit and researchand to disseminate findings.
To contribute to the development of practice, policies, proceduresand protocols relevant to the position and the health and well - being offamilies affected by dementia.
To work positively with colleagues to maintain clinicallyeffective relationships.
To participate in local clinical supervision / mentoring,providing clinical supervision to others, where appropriate.
3. Education
To participate in and/or lead, where appropriate, formaltraining events
To integrate into practice current knowledge of dementia andcaring, informed by research and other forms of evidence.
To act as a clinical role model to others and take an active rolein student nurse placements, including mentoring as appropriate.
To develop their own competence to practice through use of PersonalDevelopment Planning and continuing professional development
Tocomply with RAVEN PCN arrangements for continuing professionaldevelopment, including being aware of own development needs and how they can bemet
To maintain up to date knowledge base of resources, serviceprovision, policy context, new approaches, interventions and treatments indementia care and contribute to local policy and procedure development andreview.
To contribute to a learning environment within RAVEN PCN.
4. General
To contribute to the development of policies, procedures andprotocols relevant to post and the health and wellbeing of the client group.
