Faster Diagnosis Nurse (FDS) - Lower GI Pathway

An exciting opportunity has arisen to join the expanding Faster Diagnosis team focusing on the Lower GI patients. The post holder will be focused upon the start of the patients' cancer journey, including triage, pre-assessing, and prescribing ahead of potential diagnostic tests or first outpatient appointments.
This is a centralized service looking across all referrals into the Lower GI team in HHFT, as such primary location is flexible although cross-site presence is required for multidisciplinary engagement.

Main Duties of the Job
The post holder will accept referrals to agreed criteria and be the identified key worker for a patient/client caseload to coordinate treatment and care. They will provide advice, education, and support to patients, their carers, and other healthcare professionals.
To practice autonomously, carrying a caseload of patients referred for the suspicion of, diagnosed with, or who develop colorectal cancer. Supporting the team in the delivery of a seven-day, twenty-four-hour service (where applicable) for which you are responsible for or have been delegated the responsibility.
To work as an effective member of the wider colorectal nursing team and MDT, to forge and develop strong working relationships with cancer management and palliative care teams, this includes:
* High standards of Professional Practice to ensure quality and safety of patient care, experience, and the patient journey.
* Specialized clinical practice, knowledge, skills, and experience.
* Supporting and contributing to the team in ensuring that clinical services are delivered to a high quality of care and that all compliance requirements such as CQC and contractual requirements are achieved.
About Us
We have a well-established colorectal service at each of our main hospital sites, Basingstoke and North Hampshire Hospital and Royal Hampshire County Hospital, with two Colorectal CNS nurses based at each site. At the Basingstoke site, our Colorectal team also works closely with the Pseudomyxoma unit to support the delivery of specialist pseudomyxoma care. We would be looking for the successful candidates to join our established team to support in the delivery of care for our Colorectal patients.

Person Specification
Training and Education
* Professional UK registration e.g. NMC, HPB
* Degree level education or equivalent knowledge and skills gained through any combination of alternative study or employment experience
* Management and Leadership Development
* Experience of triaging patients
Experience and Knowledge
* Qualified for at least 3 years with 1 year within Colorectal Surgery
* Provide evidence of service development
Skills and Ability
* Clinical Skills appropriate to the relevant specialty
* Advanced communication skills including; to communicate complex, sensitive, or confidential information in an appropriate manner; to liaise effectively; to understand and disseminate multifaceted information
* Ability to work autonomously
* Experience of triaging patients
