Job Title: Social Worker – Hospital Team
Location : Cheshire West And Chester Council
Rate : £35 per hour
Liquid Personnel is currently recruiting for Social Worker to join a busy Hospital Team based in Cheshire West.
What will your responsibilities be?
In this role, you will be undertaking section 2s and 5s, facilitating hospital discharge, carrying out assessments in different wards, managing a busy caseload within an acute hospital setting, and working closely with health colleagues. You will ensure adults in hospital are discharged in a timely and appropriate manner, attend multi-disciplinary meetings, conduct mental capacity assessments leading to best interest decisions, and advise patients and families who do not meet the eligibility criteria, redirecting them to other agencies.
Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
