Main duties of the job What You'll Do Manage a dynamic caseload, monitoring patients' mental health, promoting independence and social inclusion, and supporting their recovery to reduce hospital admissions. Collaborate closely with the Operational Team Manager, Clinical Leads, and other community and inpatient services. Provide compassionate, person-centred care as part of a 9 am to 5 pm service, Monday to Friday no weekend working required Visit patients in the community, so a full driving licence and access to your own vehicle with business insurance are essential (expenses reimbursed).
