The North West London Children’s Continuing Care team is looking for a forward-thinking individual, a registered professional experienced in working with children and young people with complex physical and mental health needs.
You will join a friendly, dynamic, experienced fast-paced team to help meet the statutory responsibilities under the Children's Continuing Care National frameworks.
You will assess, case manage, and support the healthcare needs of children and young people with complex health needs.
The nature of the role is autonomous and challenging, but with the benefit of supportive colleagues.
In your role, alongside excellent assessment, communication, and coordination skills, we are looking for individuals with a strong commitment to partnership working to ensure our clients receive full care provision according to individual needs. The post holder will ensure the provision of seamless, integrated services or packages of care. Each package varies according to the health need support required, and we are looking for a professional who can think with and support families to achieve their positive health outcomes.

Main duties of the job
1. Ensure a robust assessment of individual patient eligibility and negotiation of safe and appropriate packages of care are commissioned.
2. Case managing children and young people eligible for continuing care, ensuring timely reviews are conducted and packages continue to be effectively commissioned and are cost effective.
3. Attending decision-making panels to present own cases and provide clinical information.
4. Developing strong working relationships with partner agencies.
5. To attend the team Quality Assurance/Peer Review Meetings to present cases and be responsible for the follow-up of any action as necessary.
6. To report ongoing/developing concerns relating to provider performance, updates on Corrective and Development Action Plans as necessary.
7. Reviewing, developing, and managing the process for Children's continuing care, children's personal health budgets, and specialist equipment requests.
8. Oversee the service in the absence of the team leader.
9. To ensure relevant information is provided to address complaints, investigations, and appeals.
10. To work alongside Children's safeguarding leads.

Person specification
Knowledge and experience
* Experience working as a nurse assessor or equivalent for continuing care/continuing healthcare (CHC).
* Excellent knowledge of continuing care (CC) guidance, policies, and decision-making.
* Evidence of continued professional development.
* A Degree supplemented by diploma level specialist training, experience, short courses Registered children and young people’s nurse on the Nursing and Midwifery Council (NMC) register.
* Experience of working with clinicians and/or within clinical settings to deliver excellent health outcomes for children and young people in care settings.
* Experience or a strong understanding of the policies, procedures, and practices employed by clinicians and social workers to assess and review eligible clients in receipt of continuing care, children’s social care, and SEND.
* Experience of working in the NHS and/or local authority at a relevant level.
* Demonstrated experience of delivering the best possible clinical outcomes for children and young people eligible for care services.
* Significant experience of effective work with children and young people patients/service users, families, and carers to deliver the best possible clinical outcomes.
* Experience of chairing complex meetings across multi-disciplinary teams.
* Experience of successfully managing disputes between organisations or with clients and their families, including through recourse to legal avenues of redress where necessary.
* Ability to provide and receive highly complex, sensitive, and contentious information, to negotiate with senior stakeholders on challenging and controversial issues, and to present complex and sensitive information to large and/or influential groups.
