Job summary We are looking for an enthusiastic band 6 District Nurse team leader to deliver care to the housebound population within the primary care setting. You will be caring for patients with long term conditions and complex health needs, developing your skills and knowledge, whilst working towards an integrated healthcare structure. You should hold the Specialist Practitioner Degree qualification (SPQ). You will be responsible for leading teams and liaising with stakeholders ensuring the delivery of safe effective care. You should be able to demonstrate you can make clinical decisions to provide evidence-based practice and care planning in response to clinical assessments.
