Floating Support Coordinator
Location: Manchester
Salary: £24,020 - £30,790 per annum

The Role
As a Floating Support Coordinator, you will have the opportunity to support clients through one-to-one work, care planning, risk assessment, and multi-agency working. You will also be involved in delivering group interventions and liaising with their partners to support each client's overall health and well-being. This role is based in Manchester and involves visiting clients in their homes to deliver support. Therefore, a Valid UK driving license is required.

Key Duties:
As part of their integrated team, you will collaborate with colleagues, community organisations, and stakeholders across a range of settings to help individuals maximise their sustainable recovery capital and community opportunities.

Their focus is on ensuring effective delivery of a Support package within the full integrated service model, with a focus on successful and sustainable exit from support.
