This is an exciting opportunity for a Long Term Conditions Nurse to join Rosebank Health. We are looking for an experienced, enthusiastic Respiratory Nurse to take an active role in our team, providing high quality care to our 40,000 patients.
The role will be focused on COPD and Asthma, reviewing a wide age range of patients and their respiratory health, managing acute and chronic needs.
In addition, there is potential for general treatment room duties.
Main duties of the job
The successful candidate will:
1. Undertake the role of Respiratory Nurse and collaborate with colleagues in the planning and delivery of respiratory care in line with Practice need.
2. Work as an autonomous specialist practitioner effectively coordinating the delivery of an expert service within respiratory disease, supporting health and social care professionals in accordance with local and national guidance to promote guideline compliance.
3. Work closely with the wider nursing team, clinical admin team, and Lead GP to meet quality standards such as QOF.

We have a wide clinical team including GP Partners, Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Paramedics, HCAs, Phlebotomists, Pharmacists, Social Prescribers, and a Physiotherapist, who are all well-supported by a large team of committed administrative staff.
Job responsibilities
Responsibility for Patient Care:
1. Clinically assess, manage and review respiratory patients, identify and advise on risk factors and act to prevent avoidable hospital admissions where possible.
2. Responsibility for caseloads, focusing particularly on the complex needs of patients with respiratory illness, ensuring all patients have an accurate management plan, reflecting assessments and incorporating the issues and recommendations made.
3. Practice as an autonomous professional in a self-directed manner with confidence, authority, and expertise, undertaking a comprehensive range of clinical skills within the specialist area.
4. Promote health and wellbeing, empowering patients to make lifestyle changes and self-manage, offering the appropriate health education information and advice to patients, relatives, and carers.
5. Provide expert nursing advice and support to patients, their families/carers following diagnosis and through the treatment process, identifying particularly complex clinical needs.
6. Use professional judgement and clinical experience in managing complex and unpredictable patient events, learning from situations to improve future patient care and service delivery.
7. Critically evaluate and challenge current practice and processes of respiratory care and management, identifying and initiating appropriate steps to assist with innovative solutions to modernise and develop services to achieve Practice objectives.
8. Help with the identification, development, and evaluation of clinical protocols and strategies to standards of specialist respiratory disease management whilst ensuring equity of access into the service via numerous pathways/referral routes.
9. Collaborate with IT to ensure full understanding of QOF recall systems and to ensure smooth running of clinics.
10. Collaborate with palliative care services and other professional disciplines to provide a seamless service for patients who are end of life, assisting in the management of patients in the terminal phase by joint working with community/neighbourhood healthcare professionals and palliative care services following end of life discussions and offering appropriate specialist opinion in relation to the management of respiratory illness.
11. Assist to implement nurse-led clinics for respiratory management using advanced specialist skills and knowledge to ensure consistent high standards of care.
12. Contribute to PCN meetings and projects.

Person Specification
Experience
* Experience working in a primary care setting.
* Evidence of working autonomously or as part of a team.
* Understanding and knowledge of healthcare provision in general practice, including QOF and enhanced services.
* Good knowledge of clinical IT systems such as Systm1.
Skills
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face.
* Knowledge of IT systems, including ability to use word processing skills, emails, and the internet.
* Ability to promote best practice regarding all nursing matters within your scope of practice.
* Ability to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.
* Effective time management and adaptability.
* Demonstrate personal accountability, emotional resilience and work well under pressure.
* Flexibility to work outside of core office hours.
Qualifications
* NMC Registered Nurse.
* Diploma Level or equivalent in Respiratory.
* Educated to Degree Level.
