Job Title: Adult Social Worker - Front Door Team - Stockport Location: Stockport Pay Rate: £ 30/per hour A temporary position for an experienced Social Worker is available for a 3-month contract. The role involves working with the Front Door Team and the Urgent Community Response Team. Key responsibilities include collaborating with Health colleagues to minimize hospital visits, conducting visits as needed (including same-day responses), completing Care Act and short-term assessments to promote independence at home, and providing Social Work expertise within a multi-disciplinary team. The position is office-based, and requires a clean driving license and transport. Qualifications and Experience: To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
