Job summary
We are seeking a compassionate and experienced individual to be part of our nursing team at South Bucks Hospice. This is an exciting role for an experienced registered palliative care nurse, who will be able to show development throughout their career.

This is a permanent part-time role at hours per week paying £21,827 per annum (FTE £36,379).
You will receive pro-ratad 25 days annual leave plus bank holidays increasing with length of service. We also offer to those currently a member of the NHS Pension Scheme the option to remain in the scheme or alternatively the option to join an employer contributory pension scheme. Free on-site parking is also provided.

Main duties of the job
Provision of high standards and ethical practices of patient centred palliative nursing care in line with the personalcare plan.
To act in accordance with the Nursing and Midwifery Council (NMC)Code of Conduct and in line with South BucksHospice (SBH) policies and procedures.

7. Full compliance with professional, ethical patient assessments, care plans and note taking are clear andaccurate by both self and the wider team.
8. Taking a wider view to patient care by supporting community teams in their practise of patients requiringpalliative care and referring appropriately to other in-house services and to external services.
9. Actively comply and demonstrate the implementation of good infection prevention control in accordancewith policy and procedures.
10. Work with the team, using a patient-centered approach and evidenced based practice, assess, plan,implement and evaluate patient care and co-ordinate care from initial assessment admission to discharge.
11. Administer medication and treatment as prescribed in accordance with the NMCs and the Charityspolicies and guidelines if the patient is unable to self-medicate.
12. Liaise with the multi-disciplinary team regularly to ensure timely delivery of care and foster good workingrelationships.
13. Liaise and communicate with external services District Nurses, Specialist Palliative Care nurses andHealth and Social Service Departments to arrange discharges and/or support within the community.
14. Adhere to the Care Quality Commission five key principals. Well-led, Responsive, Effective, Safe andCaring.
15. Maintain a pleasant and safe environment for patients visitors, staff and volunteers.
16. Responsible for the assessment of care needs and the development, implementation and evaluation ofpatients care plans without supervision.
17. To liaise with the Lead Palliative Care Nurse on all developments relating to patient care, staff andvolunteers.
18. Oversee the department in the absence of the Lead Palliative Care Nurse.
19. Understanding of the referral process to the Safeguarding Adults Board (SAB) Multi Agency Safeguarding Hub (MASH).
20. Support a positive learning environment for junior staff, new colleagues and students.

21. Appropriate Palliative Care Nursing Qualification and minimum of two years experience within a similar
