We are looking to recruit a clinical Supported discharge Matron to be part of the BHFT East Supported discharge service. The role will be based in WPH working with the acute discharge team. You will be expected to carry out face-to-face assessments within the acute as well as work with the hospital MDTs, including physicians, dietitians, and specialist nurses, to offer a joined-up, integrated service. This is a 1.0WTE Band 7 Permanent Post. You will join a small team of clinicians based in RBH.

Main duties of the job
* This is a 1 WTE Perm post
* You will be a core member of BHFT supported discharge service
* You will work as a key part of the MDT within both the acute and the community
* Providing a high level of clinical assessments and interventions as appropriate including indirect work as appropriate
* You will liaise with other services within both primary and secondary care to ensure those individuals with more complex needs within the acute have access to timely appropriate services to enable an integrated holistic approach.
