We have an exciting opportunity for an experienced Paediatric Nurse seeking to undertake an interesting and highly rewarding senior nurse role.
This post provides the opportunity for a forward-thinking and experienced Paediatric Nurse with extensive clinical skills/knowledge in children's complex physical needs to contribute to the co-ordination of a team of Health Care Support Workers, Nurse associates, and Staff Nurses in delivering individual packages of care.
The CPOC Team provides nursing and specialist healthcare to Children & Young People (CYP) in their own homes and schools who have an identified health need against the continuing care criteria. Support from a package of care facilitates the transfer home from hospital for many CYP and enables their families to manage their child's complex health needs within the community to achieve an active life.
The team is directly commissioned by the ICB's via the Children's Continuing Care Framework across Lancashire to deliver packages of care to children and young people with complex physical health needs. They are also commissioned to deliver support in schools and social care settings to individual children.
The post holder will ensure a high-quality, evidence-based service with clinical care pathways delivered utilizing available resources effectively in a safe and supportive manner when meeting the needs of CYP and their family.
