Job Title: Social Worker - Experienced Hospital Discharge Location: London Rate: Up to £35 per hour An exciting job role has recently become available at the North London Local Authority. They are looking for a new member for their Hospital Discharge Team. What will your responsibilities be? The team is seeking an enthusiastic individual to engage in strengths-based, person-centered assessments. This role involves managing a varied caseload, including capacity assessments, best interest decision-making, Care Act assessments, and participating in a duty rota. The successful candidate will deliver exceptional social care to adults and their carers, promoting choice and control as much as possible. Additionally, they will engage in outcomes-focused and preventative conversations that build on the strengths of individuals and their local networks.
